Our team helps care for those patients wherever they reside – in their own home, care facility, hospital or our own Cedar Valley Hospice Home – with a goal of less pain and fewer visits to the hospital.  This means more time can be spent with loved ones, making memories that will last a lifetime.

Anyone can make a referral to hospice – friends, family or a doctor. Once that referral has been made a hospice nurse will schedule a visit with you to determine hospice eligibility. When you are on the Cedar Valley Hospice program, your doctor will continue to be a part of your care along with the hospice team. Our teams of experienced professionals are available 24 hours a day, 7 days a week. We want to make every day the best day possible for you and your family.

Can I afford hospice care?

Yes, you can! We believe that everyone deserves hospice care at the end of life. Hospice fees are covered by Medicaid, Medicare and most private insurance plans. However, no one will be denied hospice care based on an inability to pay.
