Hospice is a special kind of care focused on the physical, emotional and spiritual needs of terminally ill patients and their families. Hospice care treats the person rather than the disease, focusing on the quality of life and making each day the best it can be.

At Cedar Valley Hospice, care decisions are made by the patient and family in coordination with a caring team of professionals which includes your physician. Cedar Valley Hospice offers:

  • Care provided in nursing homes, assisted living centers or hospitals, at a patient’s home or in our Hospice Home.
  • Training for families on how to care for their loved one at home.
  • Help available 24 hours a days, seven days a week.
  • Hospice care available to anyone who qualifies, regardless of age, illness or ability to pay.
  • Grief support for family members and loved ones during and after hospice care.

Having a terminally ill loved one is difficult. Cedar Valley Hospice can help support you and your loved one through the end-of-life process with the utmost care and respect.

Our hospice services are for anyone with a life threatening illness and an estimated life expectancy of six months or less. Care at Cedar Valley Hospice  is available to all those who meet our  admission criteria, and is provided regardless of age, gender, race, sexual orientation, national origin or ability to pay.

 

Cedar Valley Hospice accepts patients who meet the following criteria:

  • Those with an estimated life expectancy of six months or less
  • Anyone in the later phases of an illness, including heart disease, Alzheimer’s, cancer, AIDS, ALS, neurological and renal diseases, chronic lung disease or any other end-stage organ disease
  • The patient and their doctor have agreed on a path toward comfort care, rather than curative care.

They can be referred by a family member, friend, clergy, physician, nurse or other healthcare professional, after which our staff will contact the patient and the family. They are admitted when the patient and their physician agree that curative care is no longer an option, and care that provides comfort, pain management and family support should be in place. The patient and family are then contacted about setting up an admission meeting with Cedar Valley Hospice.

Anyone can make a referral to Cedar Valley Hospice . If the physician has not yet suggested hospice or made a referral, you can ask or contact Cedar Valley Hospice to get more information at any time. Our team can also contact your physician to discuss options and obtain orders to admit the patient to hospice care, if eligible.

Cedar Valley Hospice is able to provide care to patients and families throughout the Cedar Valley and Northeast Iowa through our four offices, plus the Hospice Home located in Waterloo. We proudly serve Black Hawk, Bremer, Buchanan, Butler, Grundy and Tama counties, as well as parts of Benton, Chickasaw, Delaware, Fayette, Floyd, Franklin, Hardin, Linn and Marshall counties. We have office locations in Waterloo, Waverly, Independence and Grundy Center. Each office has staff and volunteers ready and willing to provide a helping hand, a listening ear and place to turn for support.

Although all hospices work with terminally ill patients and have the same regulations, the actual service offerings can differ widely. Cedar Valley Hospice is an independent, not-for-profit community healthcare provider, as well as the only hospice in the Cedar Valley with a Hospice Home. We have been serving the community since 1979 with a goal of Making Each Moment Matter for the patients and families we serve.

Hospice care at Cedar Valley Hospice is available to anyone who qualifies for admission, regardless of ability to pay. Medicare, Medicaid and many private insurance plans cover hospice care. If you have additional questions, please call us at any time.

No, this is a common misconception. Hospice services are offered to anyone with a terminal illness and a life expectancy of six months or less, if the disease runs its normal course. This includes a variety of diagnoses from ALS, Alzheimer’s, heart disease, AIDS, neurological or renal diseases, chronic lung disease and other late-stage organ diseases, as well as cancer.
