Medical and Social Support for Those Diagnosed with HIV or AIDS
The Cedar AIDS Support System – CASS – at Cedar Valley Hospice provides free and confidential medical case management for persons living with HIV or AIDS.
CASS Information and Referral
Information on CASS is available to anyone, and anyone is able to make a referral. CASS case managers have access to a wide variety of information and resources and also work with many healthcare providers, community services and organizations. The goal is to assist clients/caregivers in finding medical and emotional care and also to increase the quality and consistency of financial and support services.
Individual Case Management
Whether a client has specific physical, emotional, social or spiritual needs, CASS case managers listen and identify these needs while getting to know the client. This helps the case manager make the correct referrals for the client’s needs while maintaining contact with the client.
CASS understands the confidentiality concerns of many people with HIV and is familiar with the laws protecting HIV-related information. You will be asked at your first visit about your wishes relating to sharing information with associated agencies and providers, and your wishes will be respected.
In times of crisis or emergency, CASS clients can apply for financial assistance through the Ryan White CARE Act. Housing opportunities for Person with AIDS (HOPWA) and the CASS Emergency Fund. Our direct assistance program also offers basic need items and transportation reimbursement.
CASS in the Community
CASS works with schools, community service organizations, churches and health care providers to assist, support and educate HIV-positive individuals, those participating in high-risk behavior and anyone who is interested in HIV and AIDS issues.
