CASS Case Manager (Full Time 1.0 FTE) – Waterloo, Ia
Cedar Valley Hospice is the premier hospice agency serving 15 counties with offices in Waterloo, Grundy Center, Independence and Waverly.
We are currently seeking a highly qualified Cedar AIDS Support System (CASS) Case Manager to join our team. The CASS team provides free and confidential HIV case management for people living with HIV.
The CASS Case Manager is responsible for providing supportive services to persons with HIV/AIDS and their family members/significant others through case management, community outreach and counseling.
Bachelor’s degree in Social Work, Human Services or related field preferred, with one year experience in AIDS education and/or direct services preferred. One year experience working with the health care system and community service agencies required. Excellent computer, communication, time management and interpersonal skills required. Must possess current Iowa driver’s license and a reliable vehicle. Hours for the position will be Monday- Friday from 8:00am-4:30pm, with some flexibility required based on agency/client needs. Some travel is required, mostly in Northeast Iowa.
