When care begins earlier, you benefit from better symptom management, fewer hospital visits and a trusted care team that truly knows the patient and their family. Care is provided at home, wherever the patient calls home, or at the Cedar Valley Hospice Home, which may be used for short-term care as an alternative to a hospital visit or to provide respite for the caregiver.Â

The GUIDE Dementia Care program aims to improve the quality of life for people living with dementia and their caregivers, reduce burden and strain on caregivers and prevent or delay long-term nursing home care. If you or a loved one has or suspects a dementia diagnosis, is enrolled in Medicare Parts A & B and lives at home, an independent or assisted living facility or short-term skilled stay, you may qualify for an added layer of support
