Supportive Care
at Home

LINK is the palliative care program of Cedar Valley Hospice, designed to support individuals and families living with a serious illness. A dedicated team consisting of a nurse and social worker provides in-home visits to assess needs, manage symptoms and offer guidance.

Unlike hospice care, patients in the LINK program can continue to pursue curative treatments while receiving added support. Common conditions include COPD, cancer, Alzheimer’s disease, Parkinson’s disease and heart disease.

Goals of LINK:
  • Listen to patients and families
  • Inform patients and families of resources in their communities
  • Navigate the healthcare system
  • Knowledge/educate patients and families on healthcare choices and treatments

The LINK teams consists of:
  • A nurse practitioner
  • A nurse
  • A social worker

Care decisions are made collaboratively with the patient, family and primary care physician.
